    How to Report a Safety Event

    UofL Physicians believes in safety awareness and promoting a just culture. No disciplinary actions shall be taken in retaliation for reporting safety events.

    The team member who was involved in, discovered or responded to the incident is responsible for completing the report and notifying their supervisor.

    Although not recommended, reports can be made anonymously be calling the Compliance Hotline at 877-426-6759.

    Reporting a Safety Event Helps to Identify:

    • Unsafe systems/processes
    • Areas for process improvement
    • Educational oppotunities
    • Potential trends

    How to report

    1. Login to MyULP (my.ulp.org)
    2. Click on the tab - Forms and Resources
    3. Click Safety Learning Report
    4. Complete all applicable fields, providing as much detail as possible
      1. Fields marked with a "*" are required and much be completed
    5. Click "Submit" once completed
      1. The form is automatically sent to Compliance and CNE to review

    If MyULP is down, the downtime Safety Learning Report form can be completed and scanned to Compliance.

    If team member has a work-related injury/illness and emergent care is required, complete the online Safety Learning Report AND the First Report of Injury (FROI) form. The FROI should be submitted to [email protected].

    In phrasing a conversation with another member of the team, you should consider the following...

    • Situation - What is happening with the patient?
    • Background - What is the clinical background or context?
    • Assessment - What do I think the problem is?
    • Recommendation - What would I recommend? What do I need from you?
